Private Medical Health Insurance Option

International students on F and J visa statuses may substitute the LSU Student Medical Health Insurance Plan with a private medical health insurance plan which meets all mandatory requirements set forth by the University (and the United States Department of State for J visa status holders.)

Steps to Use Private Medical Health Insurance

To use a private medical health insurance plan, please take the following steps:

  • Pick up an F-1 or J-1 Insurance Coverage Evaluation form at the front desk of 101 Hatcher Hall.
  • Have your insurance agent completely fill out and sign the evaluation form and send it back to you for your signature on the Student Signature Line
  • Submit the form to International Services. You can bring the form to 101 Hatcher Hall, fax it to 225-578-1413, or e-mail it to Janice Goodloe.
  • Repeat these steps each semester. (For more information on the form's due date each semester, please email iso@lsu.edu.) 

Important Notes

Evaluation Forms

  • Evaluation forms must be completely filled out, dated, and signed by private medical health insurance companies and submitted to International Services by the insurance deadline date at the beginning of each semester, regardless of the insurance coverage end date stated on any previously submitted and/or approved forms.
  • Copies of insurance cards, letters, certificates, etc., will not be accepted in place of the evaluation form. 

Insurance Credits

  • Please note that a credit of the insurance charge is not immediate. IS must process the evaluation form and contact the insurance carrier to verify coverage; due to this necessary processing time, the credit may take a number of business weeks. 
  • Students whose waivers are denied, or students who must submit additional information, will be contacted directly by email.
  • Students whose waivers are approved will not be notified by email. Students may check their LSU fee bill to determine if the waiver has been approved and the LSU student health insurance charge has been credited.
  • Please note that IS cannot remove an insurance charge or request a refund for previous semesters.
